In the era of personalised medicine, targeted radiopharmaceuticals play a pivotal role in diagnosing and treating various pathologies with higher accuracy and minimal adverse effects. Iodine is imperative for thyroid hormone production and thus radioactive iodine has an important role to play in the diagnosis and treatment of various benign and malignant thyroid disorders. This chapter describes radioiodine from its history, and the diagnostic and therapeutic uses of its various isotopes, to the current and future role of nuclear medicine in radioactive iodine-refractory thyroid cancers.
