CHAPTER 24: Data Integrity Audit Aide Memoire Check Access

This chapter contains an auditing aide memoire that is based on the Data Integrity Model described in Chapter 5 and should be used in conjunction with Chapter 21 (Quality Assurance Oversight), Chapter 22 (Data Integrity Investigations) and Chapter 23 (Data Integrity and Outsourcing). Note that the questions in the sections in this chapter and annotations in the figures are intended for experienced auditors to stimulate further questions and are not intended to be comprehensive; they are intended to be aide memoires rather than definitive and detailed checklists. The main reason is that checklists typically do not work and they constrain an auditor. In many parts of this chapter, the aide memoire is unusual in that the questions are presented as figures as this is easier to refer to during an audit. The bullet points and questions presented here do not always ask for documented evidence as an audit is assumed to be conducted by an experienced auditor.
